• 1. Program Administration: Upon the sale and claim submission according to program rules, the customer will be reimbursed up to $1,000. a. Purchase and installation of equipment which cost less than $1,000 will be paid at the actual cost. For example: modifications, which cost $800, will be reimbursed $800. Modifications which cost $1,500, will be reimbursed the maximum $1,000. b. To be eligible for this payment, equipment must be installed in a new or CPO vehicle purchased within the program period. c. All payments will be made directly to the retail customer. 2. Eligible Vehicles: New, unused or CPO Volkswagen models. 3. Eligible Sales Type: KOS 0 or 6. 4. Claim Submission Requirements: After reporting the sale, the following documents must be submitted directly to Volkswagen at the email or mailing address below. a. Claim Form: The customer must complete and sign the form. b. Sale Agreement: A photocopy of the signed document only. c. Modification Invoice: A photocopy of the paid invoice from the adaptive equipment company itemizing the modification that must state the customer's name, eligible VW vehicle and the VW Vehicle Identification Number (VIN)
